Quality Assurance list

 

This screen set is for you to enter data that can be tracked later to teaching/QA purposes. Increasingly we are being forced by regulatory bodies to provide evidence that what we do is worth the expense. This screen set has both categories (tick boxes) and free text section that you can detail your findings or how echocardiography influenced the procedure, or your management. This database has been set up so that we can collate de-identified information from many users onto a central server for large scale audit. You are strongly encouraged to spend the time and fill out this screen.

 

Remember that the post procedure report is actually a new echo, so you must enter the QA data in the echo that it occurs - either the Standard or the Post-Procedure echo or both.

 

Quality Assurance Data Entry Screen

 

There are 2 dropdown boxes for surgical impact and perioperative impact for each finding. This is so that you can detail more than 1 event. If there are more than 2 impacts, then enter a new finding section but use the same initial descriptors.

 

The default is Not Assessed. This is because we need to know what the denominator is - which is the number of studies where QA data has been filled in, rather than the number of studies performed.

 

 

The free text box is there for you to describe any positive findings in greater detail - a very important thing to do when you come to review the information later!
